There’s new technology at the 24 hospitals across Northeastern Ontario that will tell hospital staff if an individual patient is already getting home care services.

When patient who are part of the North East Community Care Access Centre present themselves in an emergency department, e-Notification will pass the information on to hospital staff.

Director of Business Solutions at the CCAC, Peter Taylor says this means better patient care and smoother patient transitions.

Health care professionals at hospitals can make better informed decisions on behalf of these patients knowing all their community care background information.

Taylor says the information-sharing also works once patients are discharged, so home-care services can be re-established or altered accordingly.

This information sharing project provides a consistent service across the entire Northeast….so it’s the same for small hospitals as it would be for large ones, like Health Sciences North.

The technology was installed earlier this year thanks to funding from eHealth Ontario.
